POSITION TITLE: Direct Support Professional

The Direct Support Professional (DSP) will need to be a self-motivated, highly-organized professional with a proven track record of dependability, and integrity. The purpose of this role is to provide exceptional day to day support that respects an individual’s preferences and choices, promoting independence and opportunity. Providing support through a plan of care developed by the Oregon Department of Human Services, the DSP is responsible for being responsive to the care plan and contributing to the individual's daily successes.
Essential Functions:Essential Functions of a DSP involves supporting each individual to lead a life that is centered around independence, enabling them to take as much control in their life as possible, and to exercise their rights, responsibilities, and obligations as citizens. For DSP's who work with children, enabling choice and control will involve close liaison with the child’s family.
Individual Care
- Delivery of personal care in accordance with the individuals care support plan
- Maintaining dignity and individuality of each client
- Provide personal care to meet the physical, psychological, social and spiritual needs of each individual
- Observing, assessing and reporting changes in an individual’s behaviors and activities
- Bathing, grooming, dressing, general hygiene, ambulation assistance, feeding
- General day to day housekeeping of the home
- Laundry whether in the home or at the nearest facility
- Meal preparation taking into account diet, budget, and safe food handling strategies
- Companionship to individual
- Assistance with shopping
- Working with individuals to established routines
- Escorting individual to doctor appointments, shopping and/or banking
- Support individual through their presence and accessibility
Mentoring
- Acting as a role model in performing housekeeping skills, personal care, and nutrition
- Motivating individual towards self-sufficiency and continued independence
- Encouraging a healthy lifestyle
Employment Requirements: Individuals must meet the following criteria:● Be at least 18 years of age● Be legally eligible to work in the United States● Understand the requirements of maintaining the confidentiality and safeguarding individual information● Able to pass a background check● Be literate and capable of understanding written and oral orders● Be able to communicate with individuals, health care providers, case managers, and appropriate parties to the client● Be able to respond to emergency situations at all times services are being delivered● Be certified in CPR and First Aid by a recognized training agency within 60 calendar days of activation of contracting - and maintain it every 2 years● Receive 12 hours of job-related in-service training annually● Have clear job responsibilities as described in a current signed and dated position description● If transporting individuals, have a valid driver’s license and vehicle insurance in compliance with the laws of the Department of Motor Vehicles
Additional qualifications required by applicable program rules for the staff of an agency endorsed to those rules may apply.
Work Environment:You will be supporting people in single occupancy homes, in their family home, in shared supported living environments, residential or in work, learning, and leisure services. You will be assigned to a defined geographical area-this may change from time to time.
